Introducing Elevate: AI Chart Review for Clinical Oversight at Scale

Why Chart Review Breaks at Scale

Most organizations run their chart review program on spreadsheets, email, and manual follow-up. That approach grows more fragile with every clinician, state, and specialty added.

Chart audits also produce one of the most direct signals an organization has about the quality of its care, and physicians want to use it that way. In a recent survey of Zivian collaborating physicians:

AI reads every chart before a physician does

With EHR integration, charts flow into Elevate with notes, labs, and prescriptions attached. Elevate’s AI pre-screens each one against the organization’s rubric, its policies, and the requirements of the state where care was delivered, then routes what needs clinical judgment to the collaborating physician with the relevant findings already surfaced. Elevate handles sampling, reviewer assignment, and reminders on its own.

Physicians spend their review time on the exceptions, significantly reducing the overall cost of chart review. For example, a reviewed chart in Elevate costs about $1, where manual review runs roughly $13.

The right charts, in every state

Elevate applies approximately 4,000 state collaboration and chart review rules across all 50 states, including 100% review of adverse outcomes. Sampling matches each state’s actual requirement, so nobody maintains a spreadsheet of rules and no organization over-reviews or under-reviews its way into risk.

A live view of where the workforce stands

Administrators, collaborating physicians, and the clinicians being reviewed all work in the same system, with questions, rubrics, and cadence configured to each specialty, service line, and state. Administrators see who has charts outstanding, what is past due, what got flagged for patient safety or a failing score, and how performance is trending by clinician, specialty, and site.

Clinicians log in to see their own results, acknowledge findings, and follow their own progress, which most chart review programs never give them.

Every review, score, signature, and follow-up is timestamped and exportable. A payer delegation review, state board inquiry, or M&A diligence request becomes one filtered export.
