Simply submit your referral and wait for the provider to become activated. Your referral must collaborate with Zivian for at least 60 days before you qualify for a referral bonus. We will keep you updated and ensure you are rewarded when your referral is activated!

No, anyone may submit a referral. You do not need to be a Zivian Health member to be eligible.

The referral bonus will be paid after the referred provider has successfully executed payment for two (2) Zivian invoices.
