What Is a Collaborating Physician?

A collaborating physician is a licensed MD or DO who enters into a formal professional relationship with a nurse practitioner through a written agreement that establishes expectations for clinical collaboration, physician availability for consultation, and documented oversight consistent with Ohio law and scope-of-practice requirements.

In Ohio, this agreement is called a Standard Care Arrangement.

What Does Collaboration Mean in Ohio?

In Ohio, collaboration is structured through a written Standard Care Arrangement between the NP and one or more physicians (or, in some cases, podiatrists).

Collaboration includes:

  • Defined criteria for referral and consultation

  • A process for resolving disagreements regarding patient management

  • A plan for coverage during absences or emergencies

  • Ongoing quality assurance processes, including chart review

Ohio allows remote collaboration, as long as the physician remains continuously available to communicate with the NP either in person or digitally.

Collaboration in Ohio does not require geographic proximity, but it does require structured oversight and documented quality assurance.

Ohio Collaboration Agreement Requirements

Ohio requires a written Standard Care Arrangement (SCA) for nurse practitioners engaged in practice.

The SCA must include:

  • Criteria for referral to a collaborating physician

  • A process for obtaining consultation

  • A coverage plan for emergencies or absences

  • A dispute resolution process

  • A description of services offered by the NP, including prescriptive scope

  • A plan for incorporating new technology or procedures

  • Quality assurance standards, including periodic random chart review

  • Specific provisions governing the prescribing component of practice

For the prescribing component, the agreement needs to address:

  • Parameters for prescribing medications and devices

  • Provisions for Schedule II controlled substances

  • Requirements for review of prescribing patterns

  • OARRS (Ohio Automated Rx Reporting System) compliance procedures

Both the NP and the collaborating physician must sign the Standard Care Arrangement.

Ohio Collaborating Physician Eligibility

To collaborate with an NP in Ohio, the physician must:

  • Hold an active Ohio medical license

  • Practice in the same or a similar specialty as the NP

For psychiatric-mental health NPs, collaboration must be with a physician practicing in psychiatry, pediatrics, or primary care/family practice.

Ohio limits the number of NPs a physician can supervise in the prescribing component of their practice to five.

Filing and Documentation

Ohio does not require prior board approval of the Standard Care Arrangement. However, NPs must:

  • Submit the name and business address of each collaborating physician to the Ohio Board of Nursing within 30 days of beginning practice

  • Notify the Board within 30 days of any addition or deletion of a collaborating physician

  • Notify the Board if a collaboration terminates

  • Maintain copies of previous Standard Care Arrangements for at least three years

A copy of the current Standard Care Arrangement must be retained by the NP’s employer and be available for board review upon request.

Ohio relies on reporting and documentation rather than pre-approval.

Prescriptive Authority While Collaborating in Ohio

In Ohio, an NP’s prescriptive authority cannot exceed that of the collaborating physician.

For controlled substances:

  • The NP must hold a valid DEA registration

  • Prescriptions must include the ICD-10 diagnosis code

  • The prescription must indicate the days’ supply

  • The NP may not self-prescribe controlled substances

  • The NP may not prescribe controlled substances to family members

Schedule II prescribing is limited. Outside of certain settings, NPs may prescribe Schedule II medications only when:

  • The patient has a terminal condition

  • A physician initially prescribed the medication

  • The prescription does not exceed a 72-hour supply

Ohio also requires OARRS review and specific quality assurance provisions regarding controlled substance prescribing.

Finding a Collaborating Physician in Ohio

NPs in Ohio commonly find collaborating physicians through employer relationships, hospital systems, group practices, professional networks, or collaborating physician matching platforms.

  • Meets specialty alignment requirements

  • Has capacity under Ohio’s NP prescribing supervision limit

  • Understands the obligations of a Standard Care Arrangement
